 Welcome to Humana Medicare
advantage plans. Humana offers PPO and PFFS plans. The summary of benefits tells
you some features of Humana's plans. As a Medicare beneficiary, you can choose
from different Medicare options. You make the choice. No matter what you decide,
you are still in the Medicare Advantage Program.
- Preferred Provider Organizations (PPO) - This type of plan
uses a provider network of contracted doctor and hospitals for services. You are
not required to choose a Primary Care Physician and may go to any doctor in the
network with or without a referral. Many of these plans will include part D drug
coverage. The plan will have co-pays for most services. The plans offer urgent
and emergent care when out of your service area while traveling.
- Private
Fee for Service (PFFS) -This type of plan does not use a provider network and
you may access any doctor or hospital. The doctor or hospital MUST be willing
to except the terms and conditions of the insurance carrier. They should not render
services if they do not except the plan except in emergency situations. The plan
will have co-pays for most services. The plans may include part D drug coverage.
The plans offer urgent and emergent care when out of your service area while traveling.
Please contact a representative for more information. This is not complete listing of the plans available in your service area. For a complete listing please contact 1-800-Medicare or consult www.Medicare.gov.
